Request to Carry While at Place of Employment
Immediate family members may use this form to transfer ownership of handguns to one another. Immediate family is defined as parents, children, step-children, spouses, and domestic partners.
For those not immediately related as defined above, handguns transfers must be facilitated through a NYS licensed gun dealer who has the ability to perform the required NICS check on the buyer.
Co-Registration Affidavit. This form allows for ONE other Madison County pistol permit holder to be listed as a co-registrant of handguns owned by another Madison County pistol permit holder. Use of this form does not constitute ownership and may not be used to transfer ownership. If the primary owner of the co-registered weapons wishes to transfer ownership to the co-registrant they must use the individual to individual form if they are directly related, or if not directly related, they must facilitate the sale transaction through a NYS licensed gun dealer who has the ability to perform the required NICS check.
Relinquish Co-Registration Affidavit. This form is to be used when individuals wish to terminate co-registration status of weapons listed. Use of this form does not constitute transfer of ownership.
Transferring Handguns from a Deceased Pistol Permit Holder
