Diagnostic Testing Sites for COVID-19

Who should get tested for COVID-19? 

Below outlines the criteria for when a New Yorker is eligible for diagnostic COVID-19 testing. This criteria is subject to change based on test availability, testing capacity, and for public health reasons.

Testing for COVID-19 shall be authorized by a health care provider for individuals when:
1. An individual is symptomatic or has a history of symptoms of COVID-19 (e.g. fever, cough, and/or trouble breathing), particularly if the individual is 70 years of age or older, the individual has a compromised immune system, or the individual has an underlying health condition, OR

2. An individual has had close (i.e. within six feet) or proximate contact with a person known to be positive with COVID-19, OR

3. An individual is subject to a precautionary or mandatory quarantine, OR

4. An individual is employed as a health care worker, first responder, or other essential worker who directly interacts with the public while working, OR

5. An individual presents with a case where the facts and circumstances, as determined by the treating clinician in consultation with state or local department of health officials, warrant testing, OR

6. An individual is included under other criteria set by the NYS Dept. of Health based on an individual’s geographic place of residence, occupation, or other factors that the Department may deem relevant for COVID-19 testing purposes, OR

7. Any individual who would return to workplace in Phase 1.

Madison County Testing Sites

List of Locations to get COVID-19 Diagnostic Testing
This list will be updated as testing sites change and are added. Testing locations are open to all, regardless of county of residency. 
*Madison County is working on additional drive up testing sites*

  • Walmart  (2024 Genesee St, Oneida, NY 13421) - beginning May 22, 2020
    • Monday, Wednesdays, and Fridays weekly fro 7:00 AM - 9:00 AM
      • NOTE: The site will be closed for Memorial Day, Monday, May 25, but will open Tuesday, May 26, from 7 a.m. to 9 a.m. and resume its regular schedule on Wednesday.
    • Must meet criteria

    • Site will use a self-administered nasal swab test

    • 9:00 AM – 5:00 PM
    • Must have an appointment – contact your primary care provider or call 315-363-6000

  • St. Elizabeth Medical Center (SEMC) – St. Luke’s Campus (1656 Champlin Ave, Utica, NY)
    • Must make appointment - call 315-624-6000
    • Must meet criteria 

Onondaga County Testing Sites

  • Walmart Supercenter (6438 Basile Rowe, East Syracuse, NY)
    • Open Monday through Friday 7:00 AM – 10:00 AM
    • Make an Appointment by going to MyQuestCOVIDTest.com
    • Site will use a self-administered nasal swab test
  • Syracuse Community Health Center (819 S. Salina St, Syracuse, NY)
    • Before visiting the testing center individuals should either phone their physician or call the Upstate COVID-19 regional triage hotline at 315-464-3979. 
    • Individuals do not have to have symptoms
