What should I do if I lost my COVID-19 vaccination card?

If you lost your COVID-19 vaccination card and were vaccinated in New York State*, you have two options: 

 

  • Contact your local health department or your primary healthcare provider to request a paper copy of your immunization history.
  • Apply for the New York State Excelsior Pass Plus.

*NOTE: If you were vaccinated at a Veterans Affairs facility A or another federal clinic, contact the facility directly that provided the vaccination.

What is the Excelsior Pass Plus? 

Excelsior Pass Plus supports a safe reopening of New York by providing a free, fast and secure option to present digital proof of COVID-19 vaccination. Similar to a mobile airline boarding pass, New Yorkers can store Passes digitally on their smartphones or print them out to present at participating businesses and venues. Participation in Excelsior Pass Plus is completely voluntary.

 

What information do I need to provide to retrieve my Pass?

To retrieve your Pass, you will need to provide basic personal information, including your first and last name, date of most recent vaccination, date of birth, zip code, and phone number. 

 

Why can’t I get my Excelsior Pass Plus? 

If you are having trouble downloading your Excelsior Pass Plus, there could be a few issues: 

  • You applied for the pass too early. You must wait 15 days after your final COVID-19 vaccination to apply for the Excelsior Pass Plus. 
  • You had a positive Covid-19 test result within the last 10 days, even if fully vaccinated.
  • Your contact information does not match your immunization record. Examples: 
    1. your first vaccination a PO box was provided, and the second time a physical address 
    2. your name is misspelled or 
    3. zip codes do not match
  • One or more of your vaccinations were provided out of state, or you were vaccinated at a Veterans Affairs facility A or another federal clinic that does not participate in the New York State Immunization Information System (NYSIIS).
  • If you were vaccinated at Walmart in Oneida, NY, to get your pass you may need to select Oneida County and not Madison County for your vaccination location.

Contact your local health department or your primary healthcare provider to make corrections or to update you immunization record.

 

Did your Excelsior Pass Plus Expire? 

For individuals whose pass expires after 180 days, or if yours has already expired, can update their pass by visiting https://www.epass.ny.gov/home or download the NYS Excelsior Pass Plus Wallet App, and follow the step-by-step instructions there. Passes are currently valid for up to 365 days.
