This program is overseen by the New York State Department of Health and administered by local social service districts. It reflects a coordinated plan of medical, nursing, rehabilitative, and support services provided at home to individuals who are medically eligible for placement in a nursing home.
The program is provided in the person’s home, including an adult day care facility, or in the home of a responsible adult. The program offers individuals an alternative to institutionalization.
