The Children and Youth With Special Health Care Needs (CYSHCN) Program provides services at no charge to connect families, healthcare providers, and community agencies with health information, resources, and referrals for any child with a disability and/or a suspected disability. The CYSHCN Program is for children under age 21 and their families.
Services
- Linkage to appropriate supports, advocacy, transition information, trainings and workshops, and community resources based on the needs of the child and family
- Assists families in finding doctors and specialists, insurance contacts, and mental health resources
- Provides guidance about community agencies and regional resources and disability-specific information
- Generates referrals to programs and services with parent consent
