Nominees Must Be: Honorably Discharged and A Madison County Veteran
The nominee is NOT required to be associated with or a member of any Veteran Organizations.
Please call the Veteran Service Agency located in Wampsville at 315-366-2395 and we will send you an application via mail or you can pick up in person or email Ben Barrett (WAVEM Event Chair)sstradli@twcny.rr.com
When filing the Nomination Form, DO NOT place the Veteran's name and address, only in the spaces provided below along with the name and contact information of the person or organization placing this Veteran into Nomination. This procedure will insure complete confidentiality and fairness in the blind judging process.
Please return BOTH completed forms, and any additional pages needed, to the address at the bottom on the instruction page. Please mark it "Attn: VOTY COMMITTEE" on the front. lower left corner of your envelope.
The Awards Dinner Will be held on November 7th, 2019, at Theodore's, NYS Route 5, Canastota, New York. Door will open at 5:30 PM.
