Directions: This form is to be used to ensure a complete review by the County Administrator and Compliance Officer for contracts under $10,000 prior to execution by the Chairman. The signed agreement with all exhibits, certificate of insurance and any other backup documentation should be included with the submission of this form.
Note: Committee review and Board of Supervisor approval are required for all contracts with an insurance waiver, or if requested by the Department Head, Supervisor or County Administrator.
