To request a new 911 address, please click on the link below entitled, 911 Address Application Request Form and complete the form in its entirety. Once complete, please email the form along with any required supporting documentation to 911 addressing@madisoncounty.ny.gov or mail to Madison County Real Property Office, Attn: 911 Addressing, P.O. Box 665, Wampsville, NY 13163.
In order to issue a new 911 address, the driveway location must already be established. Please verify that the driveway location has been pre-approved by the appropriate Highway Dept before submitting a request form. Also, please note, new 911 addresses may take up to 7-10 business days. In rare cases, it may be required to come out to the property and take measurements.
