Passports
Passport applications are accepted Monday through Friday between the hours of 8:30 a.m. and 3:00 p.m. Due to the complexity of the applications applicants must be in line by 3:00 p.m. Families please plan accordingly.
Items to Bring With You When Applying
- One passport approved photo (please see Passport Photo Requirements at the U.S. Department of State website). After November 1, 2016 - no eyewear may be worn in photo, excepting for medical reasons.
- Proof of Citizenship to be submitted with the application - one of the following:
- Previously issued, undamaged U.S. Passport
- Certified municipal birth certificate with registrar's seal and filing date (no hospital certificates). Birth certificates must have both parents' names on certified copy
- Consular Report of Birth Abroad or Certificate of Birth
- Naturalization Certificate
- Certificate of Citizenship
- Proof of Identity
- Previously issued, undamaged U.S. Passport
- Naturalization Certificate
- Valid Driver's License
- Current Government ID (city, state, or federal)
- Current Military ID (military and dependents)
- Please note: parents who are not U.S. citizens must present their foreign passport or their alien registration card as identification when signing for their children. Parents who are U.S. citizens can use their driver's license when signing for their children. Photo identification must be current.
- Please view the Passport Fees page for current regular service application fees. Check or money order payable to U.S. Department of State.
- A $35 execution fee for each application is required. Cash or one check per family made payable to Madison County Clerk is acceptable.
Routine processing usually take 4 to 6 weeks (subject to change).
