Children’s SPOA Universal Referral Applications
The New York State Office of Mental Health (OMH) and the Conference of Local Mental Hygiene Directors (CLMHD) have launched two new universal referral applications to simplify access to Children’s Single Point of Access (C-SPOA) and intensive mental health services for youth.
Application Documents
- C-SPOA Application Part 1 (Madison County)
- Used to refer a youth and family for C-SPOA coordination.
- Collects general demographic and mental health information.
- This universal application includes Madison County–specific information.
- C-SPOA Universal Application Part 2
- Used to refer youth to Children’s Community Residences (CCR) and Residential Treatment Facilities (RTF).
- Requires more detailed clinical information and supporting documentation.
- This application is also used to access Youth ACT services; however, Youth ACT is not currently available in Madison County.
- C-SPOA Application Part 2 – RTF Required Consents
- Required when referring a youth to Residential Treatment Facility services.
